3. has to have NoHacks in it
4. lots of underscores and dots and x's and numbers
5. has to have yt in it even if you don't do yt

This is probably the longest going username I've had on most things. I just like the look of it. But I've had tons of usernames since the name changes (Hadez, 4Life, uh_, Tyre, Luzt, 1Life (now taken by someone else :C ), Brigand, toppest) and a bunch of others.
My oldest username was Joshua4157 (username + 4 digit number I always use in emails n stuff), but I'm probably gonna stick with 5Life as its what most people remember me as.
