There are two that come to my mind.

One was when I was a little kid hiking in a national park. The hike was up a tall, windy mountain. At one point, the trail turned into a narrow, acute corner was a massive chute in the middle that one could easily fall down and die or get severely injured. There was only a two-inch wide area that I could actually walk across, and I was also an uncoordinated little nooblet. I was scared to death of it, but I did it anyway. I don't know why that trail was legal...

The second was when I was around nine. I was in a car with my grandma and we got T-boned by some kid going way over the speed limit who just ran a stop sign. I was on the side that got T-boned. The car swerved severely off the road and instead of rolling, we hit a telephone pole, preventing us from rolling and getting crushed or ejected.

I almost got hit by a car. I was scared of a bee and I ran into the street.

Once I was at a train station and someone almost knocked me on to the tracks. A train passed a couple of seconds after so it was kind of lucky I didn't actually end up in front of it.
I'm not sure if this was life threatening but I once had a bad concussion after falling off my horse and passing out for around half a minute.

Some neighbor of ours had just lost his job, and started drinking a lot and became drunk. He came outside to us, grabbed my shoulder, and with his other hand he smashed a rock into my temple.

I feel like choking, drowning, and bleeding to death are the worst ways to die. Because your basically in pain watching yourself die, trying to help yourself but you barely can't.
Also that seems really gruesome, how'd you get it out?

Well. I have but not really. I got hit straight on by a car. If it had been going any faster I would have faced extreme injuries and maybe death if it hit me the wrong way.
