Right format?

When Iapplied for Moderator on MCGamer Network, I got declinced for "wrong format." When i used the right format in the application. Can someone check that out please!?

Although only Sr. Staff+ are the only ones who can see Applications, we have a new template that was introduced not too long ago, it can be found here: http://www.minecraftsurvivalgames.com/forums/apply/ where it says Template.
