You have to make your cape here : http://www.needcoolshoes.com/banner
After that, you have to go on http://www.optifine.net/cape and select Banner.
After that put the link of your cape in the URL and it's done

Make sure you d the links correctly when linking your cape, if you accident put one space you can mess up the whole thing, just an experience from what I accidentally did

I am pretty sure it is 128 x 128 because 64 pixels is very small and would most likely be unable to be big enough for a cape.
