He was added in the patch notes as a joke. There has been a bug where the player model would sometimes appear randomly but he had normal eyes and the bug was fixed before it was stated that he was removed.

Nope.. it was not a plugin... it was in one of the first stages of Beta.. it was possible in vanilla single player minecraft..
