HOW DO I BAN SOMEONE?

Hey guys so this may seem like a noobie question but i want to report/get a guy banned from the mcsg (remains anonymous) but i dont know were to post my ban template i searched the hole site and i cant find it! Can someone please give me a link/ help me out on how to access the area to submit my ban template? Thankyou i shall always remain grateful <333
