Start a premium account? I don't know what you mean by this. BUt if you mean upgrade your account, you have to spend money. Click "upgrade" sign above. It will tell you everything.

Go to Minecraft.net and make an account. Then, either go to your local big-box store (Target, Walmart, Best Buy etc.) and buy a minecraft gift card or buy minecraft using a credit card or paypal on the actual website. Just so you know, it does cost $27 to buy. Feel free to personal message me if you have any more questions.
