Optifine.
You donate to get them.

There are capes that if you go to minecon you get a cape code to enter. also there is optifine which you pay $10 for.

Also, there is a mod.

You must have Optifine installed, it gives donators capes. They're $10 and you can donate for one (and customise it) to your Minecraft character. Note though that only you and other players with the Optifine mod can see it.

There is also mccapes are many other mods for it but like Optifine they have to also have the mod.

One exception for capes, like neonnuke said, is that going to any of the Minecons get you a special code for a cape to activate for your Minecraft character which everyone in the game can see unless they have capes disabled in the options.
