My little list:

Your Golden= full gold armour plus gold sword in hand
Mod Hunter= kill all the mods
Only the best= win a game only opening teir 2 chests
Explorer= play on all maps
Skydive= fall x amout of blocks and survive
Draw= draw a game
That Bounty= kill some one with over 1000 bounty on there head
Your enchanted= enchant somthing
