Yes, if you have a bukkit server you can download custom worlds on to it, but it manly depends on who is hosting your server. Are you hosting it or are you buying a server from a company?

Yes, you can put a map as the 'world' file in your server folder.

Yes you can upload a custom world onto your server. You can even have multiple custom worlds by using a plugin called Multiverse.
