Lagging

So lately I've been getting real bad lag when I get into a fight, and I always NCP if I click too much, or I lag too bad and they get way too many hits on me, and I die every time I get into a serious fight. I use Optifine, but that still doesn't seem to help, and I notice that it happens only randomly. Is this a server issue or one on my part?

Please help, because it's getting so bad I don't even want to play this anymore.

Have you tried reducing your video settings? Like turning off fancy graphics, clouds etc?

yeah, but it doesn't seem to affect it at all.
I did actually manage to win my last game, but I almost died in the beginning to someone with a worse weapon and lesser armor. It's like random spells of lagging.
