This problem might have occurred due to the fact you are logged in on two or more computers. It might be worth checking if you have logged out of all other computers.

Usually I either restart Minecraft, or open a single player world, and that fixes it.

You need to restart your Minecraft, or someone else is using your account, or you haven't bought Minecraft and you're using a cracked version

Utopia, try restarting your MC or force update it. This works for me, hope it works for you!
